THE SEVER DISEASE

The Sever disease is a condition in which the growth cartilage in children's heels becomes inflamed. It is generally caused by repetitive stress in the heels, it is more common in children who practice sports and manifests itself with pain in the back of the heel. In general, the pain is resolved when the growth process ends or when the physical activity that aggravates it is stopped or diminished.

ANATOMY OF THE SEVER DISEASE

The heel represents the posterior part of the sole of the foot. It consists of the astragalus and calcaneus bones and participates both in supporting the body and in ensuring balance in the orthostatic position, while walking, as well as in many other movements.

At birth, most bones are still cartilaginous, with several centers of ossification. When the heel begins to turn into bone, several areas of ossification appear. They extend to fill and replace the cartilage. One area of ossification is located in the center of the calcaneus and the second in the posterior part of it. These two areas show cartilage between them. At the age of about 16, when the growth is almost complete, the two bone areas merge.

The Sever disease occurs due to the destruction or disturbance of these areas of ossification and the cartilage between them.

HOW DO WE TREAT THE SEVER DISEASE?

1. Elevation (lifting) of the affected leg on a support or pillow, helps reduce the swelling in the heel. As the swelling recedes, the pain will also improve.

2. Stretching of the calf and thigh muscles. If the stretching exercises cause pain they will stop.

3. Avoid sports activities that cause pain. Swimming is the most appropriate sport in Sever Disease because it does not put any pressure on the heels.

4. Ice applied topically, in sessions of 5-10 minutes, 3-5 times a day, helps relieve pain and inflammation. Always put a towel between the ice and the area on which it is applied.

5. Compression by wrapping the heel with a cold bandage. Heel compression should be done by a therapist.

6. Helpful orthopedic means such as plantar proponents can help relieve pain due to the fact that it lifts the heel and reduces the pressure zones on the heel.

7. Physiotherapy helps to reduce both pain and inflammation and procedures such as electrotherapy and ultrasound are successfully used.

8. Anti-inflammatory drugs help reduce inflammation and implicitly pain. But they are recommended for short periods of time, about 7-14 days.
